Melissa – provides personalized support

Every perinatal patient has different concerns (breastfeeding, financial, trouble sleeping, etc.).   Beginning with brief surveys, Melissa gets to know each patient and tailors her conversations unique to each patient.   ﻿
By understanding the needs of each patient, Melissa:

Engages  -- two way individualized exchanges with patient
Educates – providing access to resources, content, links, hotlines, etc.
Evaluates – analyzing the improvement or worsening of symptoms
Escalates – providing two level of escalations 

Level One – highly serious, immediate call to 911/hot lines and alert to patients support team
Level Two -- worsening health concerns, alert to social worker to follow up
